totalview with mpiexec

I'm now back from vacation, sorry for my late response. The TotalView support
for MVAPICH1 is already in there. Simply use "-tv" switch of mpiexec. You should
set both environment variables TOTALVIEW and TVDSVR as mentioned in the email.
My patch is only relevant for MVAPICH2/MPICH2.
