MVAPICH 0.9.5 and mpiexec

Thanks for contributing this.  I put a note on the web page in case
anyone else happens to try out that new version of mpich on infiniband.

I also sent mail to the mvapich developers asking them to spin a new
release that would fix this incompatibility and some related scalibility
problems in the startup protocol.
